How to Submit
Nominations must be submitted via email no later than December 14, 2011, using the applicable entry form. Submit Entry Forms to crystal@leadingagetexas.org. Click the category name above for the entry form and category details.
Entry Form Prep Tips
Please answer all questions indicated on the entry form for each entry you are submitting. Be specific and use the category description as a guide to answer the questions. Provide as much detail as possible without exceeding the word count indicated. All Essay Questions should remain anonymous (this does not apply to media awards), do not use individual or organization names. Print and save entries before submitting to ensure a copy is available.
Judging and Notification
Nominations are open until December 14, 2011. LeadingAge Texas Network Representatives will judge the entries and present a slate of winners to our Board of Directors for approval. We will notify award winners early-February 2012 so honorees can make plans to attend the LeadingAge Texas Annual Meeting and Trade Show in Dallas, May 20-23, 2012, to accept their awards.
